What it is

An iodine-based liquid is injected through a vein in the arm and travels to the chest, where it blocks X-rays and makes blood-filled structures glow against everything else. That difference is the reason to choose this study. In the mediastinum, the central compartment holding the heart, great vessels, lymph nodes and esophagus, structures sit against one another at almost identical densities on a plain scan. Contrast pulls them apart: a lymph node stops looking like a vessel, a tumor's edge becomes definable, an abscess shows a rim. A doctor picks this over the plain version when the answer lives in that crowded space, or when tissue needs to be staged rather than simply found.

Why it is done

  • To characterize a mass in the center of the chest, and to see how far it extends into neighboring structures
  • To assess lymph nodes, which is central to staging a known cancer and to judging whether treatment is working
  • To look for infection collecting as an abscess, or for inflammation of structures around the heart and lungs
  • To evaluate the chest wall, pleura and vessels where enhancement is what makes the abnormality visible at all

What to expect

An intravenous line is placed first. Many centers check kidney function beforehand and ask about previous reactions to contrast, and some ask that nothing be eaten for a few hours before the injection. The dye commonly produces a warm flush and a metallic taste as it goes in, which passes in under a minute. The scan itself is as quick as the plain version; the extra time is in the preparation and a short period of observation afterward. The line is removed before leaving, and patients are usually told to drink fluids.

Questions worth asking

  • Is the contrast material billed as a separate line item on top of the scan, and is the injection charged too?
  • Was a combined study without and then with contrast ordered? That is a third code and costs more than either single study.
  • If the real question is a clot in the lung arteries, is a CT angiogram being ordered instead? It is a different code with a different price, and the two are easy to confuse on a bill.
  • Are pre-scan blood work for kidney function, and any premedication for a contrast allergy, charged separately from the imaging?
