Annual Physical, Established Patient (age 18-39)

This is the routine yearly preventive visit for an adult aged 18 to 39 who is already a patient of the practice. The chart already exists, so the appointment is about what has changed since last time rather than starting from scratch. It is the most commonly billed preventive visit code in this age range and is cheaper than the new-patient version.

What it is

A preventive visit for a young adult the practice already knows. Because the history is on file, the provider works from a difference: new medications, new symptoms, a job or living situation that changed, weight and blood pressure compared with last year's figures. The examination is brief and focused. Much of the value in this age band is not in the exam at all but in the continuity — a set of readings taken the same way, by the same practice, year after year, so that a drift becomes visible before it becomes a diagnosis.

Why it is done

  • To keep a running record of blood pressure, weight and other basic measurements in someone who is generally well
  • To catch changes in health, habits or family history that have happened since the last visit
  • To keep vaccinations current and address questions about contraception, mental health, alcohol or substance use in a scheduled setting
  • To maintain the relationship, so an acute problem later has a provider attached to it

What to expect

A short appointment, often 15 to 30 minutes. Many practices ask patients to complete a questionnaire beforehand covering mood, alcohol, smoking and sexual health, which the provider then works through. Blood work is not automatic at this age and depends on what the provider recommends. Results, if any, generally come through a portal rather than a call.
