ER Visit (level 1, minor)

What it is

This is not a procedure. It is the code for the visit itself — the evaluation, the thinking, and the decision about what to do next. Emergency departments bill that work at one of five levels, and this is the bottom one. Level 1 describes an encounter where the problem was single and self-limited, almost nothing had to be weighed against anything else, and the care could be completed without a physician at the bedside. In many departments it is the least-used of the five codes, because a problem small enough to fit it is usually one people take somewhere other than an emergency room.

Two things it does not mean are worth naming. It does not mean you were seen quickly — a level 1 patient may wait longer than anyone, because triage sorts by urgency. And it does not mean the bill is small: the hospital's facility charge for opening the door is applied at every level, and at many hospitals it is the largest single line on an emergency bill regardless of which of the five levels is assigned.

Why it is done

• A complaint that turned out to be minor and required no imaging, no labs and no medication beyond simple advice
• A wound check, dressing change or suture removal done in the emergency department because that is where the original care happened
• A prescription or paperwork matter handled at the emergency desk
• A symptom that resolved by the time the patient was seen and needed only a brief evaluation to confirm nothing further was required

What to expect

Arrival is through triage, where a nurse records vital signs and a brief history and assigns an urgency category. That category drives the order people are seen in, so a minor complaint generally means waiting while sicker patients go ahead. The evaluation itself is short and is often done by a nurse practitioner, physician assistant or nurse. Discharge usually comes with written instructions and a recommendation about follow-up. Nothing about the encounter is recorded as level 1 while you are there; the level is derived later from the documentation.

Questions worth asking

• What is the emergency department facility fee on its own, before any professional charge? It is charged at every level, including this one.
• Was a physician charge added even though a nurse practitioner or physician assistant provided the care, and is that clinician in network with my plan?
• Is the emergency physician group contracted separately from the hospital? A hospital in your network does not guarantee the clinician billing you is.
• If the same care was available at an urgent care clinic in this health system, what does that setting charge for the equivalent visit?
• Can I get an itemized bill rather than a summary? A level 1 visit should have very few lines on it, and extra lines are worth questioning.
