Annual Physical, Established Patient (age 40-64)

The yearly preventive visit for a patient aged 40 to 64 who already sees that practice. By this age the chart usually holds several years of blood pressure readings, lab values and screening dates, and the visit is largely about the direction those numbers are heading. It is often the appointment where chronic conditions and preventive care get discussed together.

What it is

A preventive examination for a middle-aged patient with an existing record at the practice. What distinguishes it from the same visit in a younger adult is the amount of prior data in play. A provider is comparing this year's blood pressure, weight and lab results against a column of previous ones, checking which recommended screening tests have been completed and which are outstanding, and reviewing a medication list that by this age is often not empty. The physical examination itself is a small part of the appointment.

To track trends rather than single readings — a blood pressure or a cholesterol value means something different in the context of five previous ones
To review long-term medications and how well existing conditions are controlled
To keep track of which screening tests have been done and to arrange referrals a provider recommends
To discuss vaccinations appropriate for the age group

What to expect

Practices often schedule fasting blood work shortly before the appointment so the results are available to discuss in the room. The visit itself is usually 20 to 40 minutes. Where a chronic condition needs attention the same day, some practices handle it in the same appointment and others ask for a separate one — which is a billing decision as much as a clinical one. Referrals for screening tests are typically arranged at the end.

Are tests done in the office that day — an EKG, a urine dipstick, a vision check — bundled into the visit price or billed separately?
